companies, we do not sell or rent the information

INFORMATION WE COLLECT
We collect the personal information that you
give us and information about the products
and relationships you have with us, so that
we can serve you, including offering products
and services to you. While most information
is collected at the time of application or claim,
additional information may be required if you
request changes to your contract or other service.
It includes, for example:
- Information we receive from you on applications
or other forms, such as your name, address,
telephone number, Social Security number,
health, financial, employment history, avocations
or other personal characteristics. We may also
gather information from you electronically, by
telephone and in writing.
- Information about your transactions with us, our
affiliates or others, such as your life insurance
or annuity coverage, premium payment and
investment history.
We also collect data others give us about you, for example:
- Information you authorize us to obtain, such
as medical professionals' and hospital records.
We may gather information from any required
medical exams or tests. Medical information we
receive about you will not be disclosed without
your authorization, except as required by law.
- Information from third parties such as adult
family members, employers, and other insurers.
- Information we receive from consumer reporting
agencies, such as your credit-worthiness and
credit history. This type of information will be
shared in appropriate situations and only to the
extent required for necessary business purposes.
PROTECTING CUSTOMER DATA
We maintain physical, electronic, and procedural
safeguards to protect your personal information
from unauthorized access and anticipated threats
or hazards. We maintain strict policies and
procedures to protect the confidentiality of your
information during all stages of your relationship
with us. We continually review our policies and
practices, monitor our computer networks and
test the strength of our security in order to help
us ensure the safety of customer information.
Access to your information by employees and
other representatives is restricted to those
individuals having a business need for such
information. If your relationship with us ends, we
will continue to handle data about you the same
way we handle customers' personal information.
CONFIDENTIALITY OF MEDICAL
INFORMATION
We understand that you may be especially
concerned about the privacy of your medical
information. We want to assure you that we
do not sell or rent to anyone your personally
identifiable medical information, nor do we
disclose it to others for marketing purposes. We
only use and share personally identifiable medical
information for purposes of underwriting
insurance, administering your policy or claim,
and other purposes permitted by law, such as
disclosures to insurance regulatory authorities, or
in response to legal process.
SHARING DATA
We regard all your personal information as
confidential. However, in the course of conducting
our business there are circumstances in which
we may disclose to other parties information
we have about you. These disclosures are only
made as permitted by law, such as disclosures
to our reinsurers, administrators, consultants,
and regulatory or governmental authorities. In
addition, our agents,

representatives, and others assisting them have
access to the personal information needed to
administer your business and provide service to
you. They may also use this information to advise
you of additional financial products and services
offered by Ozark and its affiliates.
We may disclose the above categories of non-public
personal information about you to non-affiliated
business partners in order to complete requested or
necessary business transactions on your application,
policy, annuity or mutual fund account, or in
order to comply with legal requirements such as
subpoenas or tax and fraud reporting.
Periodically we may work with outside firms to help
us develop our products, services, and marketing
approaches. As permitted by law, we may share
with such firms certain identifying information
and non-medical transaction information, as
described above. Whenever we hire other parties
to help us conduct our business, we expect them
to maintain the confidentiality of your personal
information and abide by all applicable privacy
laws. Further, we do not authorize them to use
or disclose your personal information for any
purpose other than the work they are performing
on our behalf or as required by law.
